2. Remember the amount you entered in this form as you need it to complete the payment page

3. When done - review entries then if OK click SUBMIT - a new page appears, this is the Payment Options Menu page.

4. Choose your mode of payment - Check by Mail or Online Payment - and double-check entries

 

5. Click ADD TO CART - The PayPal payment page appears.  Select your desired membership.
